People Say they submitted the form, but I am not getting it

Today the 2nd person reached out to say they were frustrated that the form did not submit, and when they went back, the form was blank.

I cannot afford to miss any forms, how do I fix this, and how do I see people who started the form?

There are no instructions or guidance anywhere.
